The following information will be used to calculate and to mail the form. 1) The number of months of service for which payment is made; 2) The dates for which the employee served; 3) The amount of retired pay to be paid to the employee; 4) All deductions made for any and all retirement, survivor, disability, special, and other benefits; and 5) The amount of money needed to provide each employee with retirement, survivor, disability, special, and other benefits. The number of months of service to which payment is directed will be the same as the number of months of service specified in the certificate of service given to the employee. The employee must complete the forms only when serving, without interruption, in the same role for which they were certified during the year, and they must submit the original and two copies of such certificate. The forms must be mailed from a single address and the return envelope used should have the company name and the name of the person to be paid in the address to which it will be mailed. It is the employer's responsibility to ensure these forms and the necessary documents are mailed on the proper address. If you do not wish to accept or pay these forms, they may be mailed to you as they are not required by law to be submitted. This form is to be completed on a reverse side. To find the reverse side of the form, locate the name of the person you wish to be reimbursed for, the name of the person to whom you wish to provide additional or supplemental retirement, survivor's, and disability benefits, and then the appropriate section from Table 1.
